Overview / Abstract:
In this application-based continuing pharmacy education activity Dr Michael Smith, PharmD, CACP discusses challenging topics in anticoagulation).
Target Audience:
Pharmacists who are interested in the management of patients on anticoagulation therapy and/or who are interested in starting pharmacy services for patients on anticoagulation therapy
This activity is not accredited for technicians
Learning objectives:
At the conclusion of the activity, pharmacists will be able to:
• Discuss managing techniques for challenging patient types including alcoholism, pregnancy, and patients with Antiphospholipid Antibodies Syndrome
• Describe the discrepancy between POC testing and laboratory measurement of INR
• Discuss the evidence for 12 week follow up visits and how to determine which patients are appropriate

• Evaluate anticoagulation therapy for selected challenging cases
