Heart failure (HF) is a chronic condition that requires long-term commitment on the part of the patient in adhering to an often-complex therapeutic and monitoring regimen, including lifestyle modifications and pharmacotherapy, and on the part of the clinician in maintaining knowledge of the most current therapies and devices to maximize efficacy, minimize side effects, improve monitoring, and reduce hospital readmissions. New treatments and technology have shown promise in improving outcomes in patients with heart failure, however compliance with treatment as a result of lifestyle, cost, and communication barriers, as well as monitoring heart failure symptoms at home remains burdensome for some patients and caregivers. These factors can contribute to hospital readmissions. The entire heart failure team must be involved and strategize with patients to reduce readmission rates and keep patients stable.
